The Basics of SMP

8 questions

Scalp micropigmentation — commonly referred to as SMP — is a non-surgical appearance restoration procedure based on a process called follicular replication. Using a specialized needle and carbon-based pigment, tiny impressions are deposited into the upper layer of the scalp, each designed to replicate the natural appearance of a hair follicle.

The result is the visual impression of a defined hairline, fuller density, or camouflaged scarring — without surgery, without implants, and without downtime.

SMP is not a traditional tattoo, though the equipment shares surface similarities. The technique, needle depth, pigment composition, and artistic approach are fundamentally different.

The needles used in SMP are specifically engineered for follicular replication — finer, more precise, and designed to deposit pigment at a shallower depth than a traditional tattoo needle. The pigment is carbon-based and formulated specifically for scalp micropigmentation — designed to heal naturally, fade gracefully over time, and replicate the soft, matte appearance of a real hair follicle rather than the saturated, deep-set look of traditional tattoo ink.

Traditional tattoo ink is designed to stay bold and permanent deep in the dermis. SMP pigment sits in the upper dermis — close enough to create a realistic follicular impression, shallow enough to allow for natural color evolution and touch-up refinement over time.

“The result is not a tattooed scalp. It is a restored one.”

SMP is semi-permanent. The pigment is deposited into the upper dermis and will naturally lighten over time as the skin regenerates. Most clients find the result remains clear and natural-looking for several years before a touch-up is needed.

Touch-up timing depends on a number of individual factors: skin type, sun exposure, lifestyle, and how the original sessions healed. When a touch-up is needed, it is typically a single session rather than a full replication of the original plan.

This is by design — it allows for refinement and adjustment as your scalp, preferences, and lifestyle evolve over time.

The goal of every session at Sonoma County SMP is a result that looks natural in real life — not just in photos, and not just under controlled lighting. That distinction matters.

A well-executed SMP result should be indistinguishable from a naturally close-shaved head or naturally denser hair. The key factors are pigment color matching, dot size, placement precision, and hairline design — all assessed and planned during the consultation before anything touches your scalp.

We design for real life: natural daylight, everyday settings, and close-range interactions. That is what a natural-looking result means here.

No. SMP has a much wider range of applications than the shaved-head look commonly associated with it.

  • Density fill adds the appearance of thickness to thinning hair without requiring you to shave your head
  • SMP for women addresses thinning along the part line, crown, or overall scalp — subtly and naturally
  • Scar camouflage blends FUT strip scars, FUE extraction marks, and other scalp scarring regardless of hair length
  • Post-transplant support completes the result of a hair transplant by addressing donor area scarring or density gaps
  • Hairline design can restore or refine a receding hairline while maintaining existing hair

Each of these is discussed in full during the consultation, where we build a plan specific to your situation.

Yes. SMP can be performed on all skin tones and Fitzpatrick types. Pigment color is custom-matched to your natural hair color and skin tone at the consultation — and adjusted session by session as the scalp heals and settles.

Some skin types require more careful technique or additional sessions to achieve the ideal result. This is assessed in the consultation and planned for accordingly.

SMP can be effective for various forms of alopecia, including alopecia areata (patchy loss) and alopecia totalis or universalis (full scalp loss). The approach and expected result vary significantly depending on the type and progression of alopecia involved.

Candidacy, realistic outcomes, and the appropriate plan for your specific situation are all discussed during the consultation. We will give you an honest assessment of what SMP can and cannot do for your particular presentation.

SMP does not regrow hair. It is an appearance restoration procedure — one that creates the visual impression of hair where it has thinned or receded, or camouflages areas of scarring or loss.

Hair loss medications work by attempting to slow or reverse the biological process of hair loss. Hair transplants surgically move existing hair follicles. SMP restores appearance through precise pigmentation — without surgery, without pharmaceutical side effects, and without waiting for regrowth.

These approaches are not mutually exclusive. Many clients who have had transplants later turn to SMP to complete their result. SMP can also be a first step for clients who want to address appearance now while considering surgical options.

You may have seen scalp micropigmentation described as a "hair density tattoo" or "hairline tattoo" online. While those phrases get used, they undersell what SMP actually is and overstate its similarity to traditional tattooing.

SMP uses carbon-based pigment deposited into the upper dermis — specifically engineered to replicate the soft, matte appearance of a real hair follicle. Traditional tattoo ink sits deeper, stays bolder, and behaves completely differently over time — including shifting to blue or green tones that SMP pigment is designed to avoid.

So technically — yes, it involves pigment and a needle. But calling SMP a hair density tattoo is like calling a scalpel a kitchen knife. Same general category. Completely different discipline, technique, pigment, and result.

At Sonoma County SMP we use equipment and pigments sourced specifically for scalp micropigmentation — not adapted from general tattooing. The artistry required is its own discipline.

Still not sure if SMP is right for your situation? The consultation is the right place to find out — honest, pressure-free, and specific to your scalp and goals.

The Process

7 questions

Most scalp micropigmentation plans involve two to three sessions. Each session builds on the previous one — layering pigment impressions carefully to create natural depth and density as the scalp heals between appointments.

The exact number depends on coverage area, complexity, and how your scalp responds to treatment. This is assessed and planned during the consultation before any sessions are scheduled.

A complete plan is never rushed. The best results are not done in a single sitting — they are built methodically, session by session.

Sessions typically run two to four hours depending on the coverage area and complexity of the plan. Sessions are spaced approximately ten to fourteen days apart — this spacing allows the scalp to heal and settle properly between each round of work.

Rushing the timeline between sessions produces worse results. The spacing is not arbitrary — it is how the process achieves depth and natural-looking layering that a single session cannot.

Most clients describe the sensation as mild to moderate — similar to light scratching or a mild stinging sensation. The scalp has varying sensitivity depending on the area being treated, with the hairline and crown often feeling slightly more sensitive than the sides.

A topical numbing agent is applied prior to each session to reduce discomfort. Most clients find the process very manageable and often settle in comfortably well before the session ends.

The consultation is a conversation — not a sales appointment. Before any procedure is discussed or scheduled, we talk through your scalp, your goals, your history, and your expectations.

We assess candidacy honestly, discuss realistic outcomes, and determine whether SMP is the right solution for your specific situation. If it is, we build your personalized plan together. If it is not, we will tell you that too and point you in a better direction.

Consultations are available in-person at our Santa Rosa studio, by phone, or virtually. No obligation, no pressure, no commitment required.

Hairline design is a deliberate, collaborative process — and one of the most important parts of the entire plan. Every hairline decision is made with your input, your age, your face shape, and your lifestyle in mind.

We do not design hairlines for photos. We design hairlines that look right in person — in every light, at every angle, at every age. Nothing is placed on your scalp without your approval of the design first.

There is no significant downtime. Most clients return to normal activity the same day or the following morning. The scalp may appear slightly red or sensitive immediately following a session — this typically resolves within 24 to 48 hours.

There are specific aftercare instructions to follow in the days after each session. These are provided in detail and followed up personally between sessions.

Not necessarily. Whether a shaved head is required depends entirely on which service you are receiving and what result you are looking for.

For a full buzz-cut look or clean hairline replication, a shaved or very close-cropped scalp is needed. For density fill, scar camouflage, women’s SMP, or post-transplant support, shaving is often not required — the work is done between or beneath existing hair.

Your specific situation determines what preparation is needed. That is discussed during the consultation.

Results & Expectations

6 questions

Full healed results are typically visible six to eight weeks after your final session. A touch-up assessment is scheduled after healing is complete. Some clients need minor refinement at that point. Some do not. Either way, the work is reviewed before being considered finished.

The shedding phase is a normal, expected part of the healing process — and one of the things we make sure every client understands before their first session so it does not catch them off guard.

In the first week or so after a session, pigment will appear to lighten significantly. It is not the result fading — it is the outer layer of skin naturally regenerating and shedding as part of the healing process. The pigment deposited in the upper dermis remains intact beneath.

The true settled appearance becomes visible after four to six weeks. We check in personally between sessions to make sure clients are progressing normally and are not concerned.

This is one of the most common reasons new clients contact us with anxiety after their first session. It is completely normal, temporary, and expected.

Most clients enjoy a clear, natural-looking result for three to five years before a touch-up becomes noticeable or desired. That range varies based on skin type, lifestyle, and sun exposure.

When a touch-up is needed, it is typically a straightforward single session. The gradual fading of SMP over time means the result never looks stale or outdated — and a touch-up can be an opportunity to refine the work as your goals evolve.

Yes — with some nuance. SMP is semi-permanent, meaning it does fade over time on its own. Laser removal is also an option for accelerated correction, though its effectiveness varies based on the pigment used and how deeply it was deposited.

SMP repair and correction — addressing unsatisfactory work done elsewhere — is a service we offer. If you have had SMP done previously and are not satisfied with the result, bring that conversation to the consultation where we can assess what correction is realistic.

No. SMP does not affect existing hair follicles or natural hair growth. The pigment is deposited into the upper dermis — well above the depth where hair follicles reside. Your existing hair continues to grow normally after every session.

When SMP fades naturally over time, it softens — the impression lightens gradually without turning blue, green, or any other unnatural color. This is the result of using carbon-based pigment specifically formulated for SMP, designed to fade naturally rather than shift color the way traditional tattoo ink can.

Natural fading typically signals that a touch-up session is approaching, not that anything has gone wrong.

Healing & Aftercare

5 questions

Detailed aftercare instructions are provided at the end of every session. General principles for the days immediately following each session:

  • Keep the scalp dry for the first four days — no washing, swimming, or heavy sweating
  • Avoid direct sun exposure on the treated area
  • Do not apply any products to the scalp unless specifically recommended
  • Do not pick, scratch, or rub the treated area during healing
  • Avoid saunas, steam rooms, and extreme heat

After the initial healing window, a light moisturizer is recommended to keep the scalp hydrated and support long-term pigment quality. Sunscreen or UV-protective hats are recommended for ongoing scalp care.

We recommend avoiding heavy exercise — anything that causes significant sweating — for the first four days following each session. Sweat can introduce bacteria and moisture to the treated area during a critical early healing window, which can disrupt pigment settling.

Light activity is generally fine. After the four-day window, normal exercise can resume. If you are planning around training or athletic events, mention it during the consultation and we will factor it into your session scheduling.

Yes. UV exposure is one of the primary factors that accelerates pigment fading over time. We recommend protecting the scalp from direct sun exposure — especially in the weeks immediately following a session — using SPF or UV-protective headwear.

Clients who spend significant time outdoors may find that their SMP requires touch-ups on a slightly shorter timeline than clients with lower UV exposure. This is normal and something we account for when planning your long-term maintenance schedule.

Long-term SMP care is simple. The main principles:

  • Keep the scalp moisturized — a light, fragrance-free moisturizer applied regularly helps maintain skin quality and pigment appearance
  • Protect from UV exposure — consistent sun protection extends the life of the result significantly
  • Avoid harsh chemical treatments on the scalp
  • Schedule a touch-up assessment when pigment begins to visibly lighten or soften beyond your preference

A complete aftercare guide and recommended product list are provided at the end of your final session.

Some degree of redness and mild sensitivity immediately following a session is completely normal. This typically resolves within 24 to 48 hours.

If you experience unusual swelling, persistent irritation, or anything that concerns you after a session, contact us directly by phone. We follow up personally between sessions and take any post-procedure concern seriously.

Significant adverse reactions to SMP are rare. The pigments used at Sonoma County SMP are specifically formulated for scalp micropigmentation and selected for safety and biocompatibility.

Cost & Financing

5 questions

Scalp micropigmentation pricing depends on the area of coverage, the number of sessions required, and the complexity of the work involved. No two plans are identical, so there is no single price that would be accurate for your specific situation.

For context: SMP across the Bay Area and Northern California typically ranges from around $1,200 for focused density work to $4,500 or more for full scalp coverage from established specialists. Sonoma County SMP pricing is competitive within that range and reflects the training, technique, and personal attention each client receives.

The consultation is where you receive an accurate quote — specific to your scalp, your goals, and your timeline. That quote is valid for 30 days.

Yes. It's complimentary, with no obligation to move forward. It is simply an honest conversation about your scalp, your goals, and whether SMP is the right solution for your situation. If it is, we build a plan and quote together. If it is not, we will tell you.

Yes. Financing is available through Cherry — a straightforward, no-pressure payment plan designed to make your investment manageable without affecting the quality of your care. Applications are processed quickly and directly through the Cherry platform.

Appearance restoration should be accessible. If the upfront cost is a concern, financing is worth exploring. We are happy to discuss payment options during the consultation.

In most cases, no. Scalp micropigmentation is considered a cosmetic procedure and is not typically covered by health insurance. Some flexible spending account (FSA) or health savings account (HSA) plans may cover it in certain circumstances — check with your plan provider directly.

For clients receiving SMP for medical reasons such as alopecia or scar camouflage following surgery, it may be worth checking with your provider, but we cannot make any guarantees about coverage.

A post-healing touch-up assessment is included as part of your plan — scheduled at six to eight weeks after your final session to review the healed result. Any minor refinements identified at that assessment are addressed without additional charge.

Long-term touch-up sessions — needed years later as the pigment naturally softens over time — are priced separately. Pricing for maintenance touch-ups is discussed during the consultation so there are no surprises down the road.

Am I a Candidate?

5 questions

SMP is a strong fit for clients looking for a natural-looking, non-surgical appearance restoration solution that addresses hairline recession, thinning, density loss, scalp scarring, or hair loss of various kinds. It works for men and women, across a wide range of ages, skin tones, and hair loss stages.

It is especially well-suited for clients who want a meaningful improvement to their appearance without surgery, without ongoing pharmaceutical intervention, and without a lengthy recovery period.

SMP may not be the right solution for clients who:

  • Have certain active skin conditions on the scalp (psoriasis, eczema in the treatment area, active dermatitis)
  • Are undergoing chemotherapy or certain medical treatments that affect skin healing
  • Have unrealistic expectations about what SMP can achieve — including expecting it to replicate long hair or growing hair
  • Are in a very early stage of hair loss that does not yet benefit from SMP intervention

These are exactly the conversations that happen honestly in the consultation. We will never move a client toward a procedure that is not genuinely right for them.

Yes — and SMP is one of the most effective ways to complete and enhance the result of a hair transplant. Do not think of SMP as a competitor to hair transplants — it goes hand in hand with them.

Many transplant clients turn to SMP to camouflage visible FUT strip scarring or FUE donor area extraction marks, to add density between transplanted grafts, or to refine a hairline that did not achieve the original desired result. SMP is typically scheduled after full transplant growth has matured — usually 12 to 18 months post-procedure.

Yes. Hair loss is not exclusively a men’s concern, and SMP for women is a meaningful part of the work done at Sonoma County SMP. Women experiencing thinning hair, a widening part line, alopecia, or overall density loss can benefit significantly from carefully placed scalp micropigmentation.

Results for women are designed to be completely natural — adding the appearance of density without altering or affecting existing hair. The process is private, discreet, and consultation-first. A shaved head is not required.

SMP repair and correction is a service we offer. Many clients arrive with unsatisfactory work from other practitioners — pigment that has turned blue or green, hairlines that are unnatural or overly aggressive, or uneven coverage.

The correction process depends entirely on what was done previously and what is realistic to achieve. Some corrections are straightforward. Others require laser lightening or removal prior to any new work. All of this is assessed honestly in the consultation.

If you have had SMP done and are not happy with the result, bring that conversation to us. We will give you a straight assessment of what is and is not possible.

Yes. Sonoma County SMP is registered with Sonoma County Environmental Health, operates in full compliance with California’s Safe Body Art Act, and maintains Bloodborne Pathogen Certification.

Single-use needles and a sterile clinical setup are standard at every session. Your safety is not a selling point — it is the baseline every client deserves.

Sessions need to be spaced ten to fourteen days apart to allow proper healing between each one — so a single short trip cannot compress a full plan into consecutive days. However, we work with out-of-area clients to plan a realistic timeline.

A typical travel plan: arrive for Session 1, spend the days between exploring Sonoma County (wine country, the coast, the redwoods — all within 45 minutes of the studio), then return for Session 2. Full healed results would be assessed at the six to eight week mark via photos or a third visit.

Santa Rosa is a genuinely excellent destination — not just a destination-of-necessity. Details on visiting the area are on our Visiting Santa Rosa page.
